Our Redeemer Lutheran School Forms
Through connecting faith and 21st century academics, students of Our Redeemer will be equipped to excel in any classroom beyond our walls and be faithful servants in the church and community for generations to come.
The following are links to forms for Our Redeemer Lutheran School. The forms are grouped according to their purpose, with a description of when you need to submit each form. Submit all forms to the school office – if you do not see the form you need, please call the school office at 262-728-6589.
School Handbook – this is the school handbook, containing information pertaining to all Our Redeemer students and families.
PK Handbook – this is the Preschool Handbook, containing information pertaining to PK3 and PK4 students and families.
Field Trip Driver/Chaperone Forms
Background Check Instructions – this describes our background check policy, and contains a link to the Protect My Ministry background check website. All school volunteers/drivers/chaperones need a background check on file.
Certified Driver Form – this form must be filled out if you plan to drive for a field trip, and submitted to the school office with a copy of your current driver’s license and proof of insurance.
Physical, Dental, and Medication Forms
Physical Exam Form – this form must be filled out by your child(ren)’s physician for all new PK students, for all students entering Kindergarten (even if they attended PK at Our Redeemer), and all students entering 6th grade.
Immunization Record Form – this form, or a printed immunization record from your child(ren)’s physician, must be submitted for all new PK students, for all students entering Kindergarten (even if they attended PK at Our Redeemer), and all students entering 6th grade. *If you do not plan to immunize your child(ren) due to health, religious, or personal convictions, there is a section on this form for you to sign to that effect.*
Dental Exam Form – this form must be filled out by your child(ren)’s dentist for all students entering Kindergarten (even if they attended PK at Our Redeemer).
Free Dental Exam Letter – this letter is from Dr. Ben Johnson, who will provide free dental exams to any student entering Kindergarten at Our Redeemer to satisfy the dental exam requirement.
Over-the-Counter Medication Permission and Instructions – this form must be on file for any student who is allowed to take over-the-counter medication during the school day; the medication is kept in the school office.
Prescription Medication Permission and Instructions – this form must be on file for any student who must take prescription medication during the school day; the medication is kept in the school office.
Technology Acceptable Use Policy and Form – this must be filled out each year for all Kindergarten – 8th grade students; a student will not be issued their school Google account and Chromebook until this form is on file.
Electronic Device Policy and Form – this policy describes what electronic devices are allowed (or not) at school; fill out the form if you child(ren) will be bringing an allowed device to school.
Explanation of Scrip Program – this is not a form to fill in, but describes the Scrip program and its benefits in detail.
Scrip Enrollment and Backpack Waiver – submit this form to enroll in the Scrip program; you can select to pick up your Scrip cards in the church office, or have them sent home with your ORLS child(ren).
Sunday School Forms
Kingdom Kids Registration – this is an online form to register for Kingdom Kids (PK3 – 5th grade) for the 18-19 school year.
F.B.I. (Faithful Bible Investigators) Registration – this is a form to register for F.B.I. (6th – 8th grade) for the 18-19 school year.
Hot Lunch Forms
Household Application for Free and Reduced Price Student Meals – submit this form if your child(ren) have not been automatically qualified to receive free or reduced price hot lunches, and you would like to apply to receive free or reduced price hot lunches. Check with the school office to find out if your child(ren) qualify automatically before filling out this form.

Additional School Forms
Commitment Form – this form must be filled out each year for all Kindergarten – 8th grade students (one form per household).
